This notice describes how medical information about you may be used and disclosed, and how you can get access to that information. Please review it carefully. A printed copy is available at our front desk.
How we may use your health information
We use and disclose your protected health information to provide your care, to obtain payment for that care, and to run our practice. Examples include discussing your case with another provider you have asked us to coordinate with, or documenting your visit in your chart.
Disclosures that require your authorization
Most uses of your information beyond treatment, payment, and health care operations require your written authorization. That includes any marketing use and any sale of your information, which we do not do. You may revoke an authorization in writing at any time.
Substance use disorder treatment records
To the extent that we have substance use disorder patient records about you that are subject to federal confidentiality rules (42 CFR Part 2), we will not use or share information in those records in civil, criminal, administrative, or legislative investigations or proceedings against you without (1) your written consent or (2) a court order and a subpoena. If we ever contact you for fundraising using Part 2 information, we will give you clear and obvious notice in advance and a choice about whether to receive those communications.
Your rights
- Inspect and obtain a copy of your health record.
- Request a correction to information you believe is incomplete or inaccurate.
- Request confidential communications, such as contact at a specific phone number.
- Request restrictions on certain uses and disclosures.
- Receive an accounting of certain disclosures we have made.
- Receive a paper copy of this notice on request.
Our responsibilities
We are required by law to maintain the privacy of your protected health information, to give you this notice of our legal duties and privacy practices, and to notify you if a breach occurs that may have compromised your information.
